@Manny Stay tune for more patches.. I haven't had the time to get anything past 1.16 since that was the last snapshot the internet archive had. Blizzard no longer reveals their ftp structure so I'm gonna see if I can piece any new version names for later released patches.

Does anyone know if there's a way, with multiple 16:9 displays, to lock my D2 on one window in fullscreen be it bordered or not? I can't seem to get it to work. With fullscreen the cursor can move to the other display and I can't get a maximized windowed mode working.

Yes, that is how I used to play before I bought my 4:3 monitors. Windowed full screen with the mouse locked in. I had black borders on the side, but I retained the 4:3 aspect ratio and locked cursors, which is my preference.
Are you running CNC-DDRAW? It comes bundled with with Cactus, and it allows for you to unlock the cursor with Ctrl+Alt, otherwise the cursor stays inside the game on your main monitor.
Below I will paste a screenshot of the main section of my config file (ddraw.ini). Perhaps it will be of some help. Let me know how it goes or if you need any help.

Unfortunately that did not work, but I messed around with some of the settings on plugy eventually fixing it. Your reply set me on the right path though so thank you!
